Provide training to staff in health related areas,
coordinatemedical services, communicate with medical
professionals, administer medications, maintain and update
health records, and interpret medical reports, all in a
caring, positive residential setting. Current NYS RN license
and valid NYS driver’s license with acceptable driving
record required. Experience with people with disabilities

NOTICE OF
FORMATION OF
LIMITED
LIABILITY COMPANY
Notice of formation of Limited Liability Company under New
York Limited Liability Company Law, ("LLCL"). The name of
the Limited Liability Company ("LLC") is Cadosia Heights,
LLC.
The date of filing of the Articles of Organization with the
Secretary of State is January 16, 2013.
The County within the State of New York in which the
principal office of the LLC is located is Delaware.
The secretary of the State of New York is hereby designated
as agent of the LLC upon whom process against it may be
served. The address to which the Secretary of State shall
mail a copy of any process against him or her is 38 West
Main Street, Hancock, New York 13783.
The character or purpose of the business of the LLC is any
purpose allowed by law.
A thank you note to all family and friends for your
concerns during my hospitalization in Raleigh, North
Carolina. All of the cards and flowers that were sent were
greatly appreciated. A small community is a great place to
be from because you know people are very caring. We came
home after a week’s stay and more cards, meals, and flowers
arrived at the house. Our sidewalk is shoveled and our
mailbox was repaired just because we have great friends and
neighbors. All the prayers were greatly appreciated as there
is one Spirit that holds us in the palm of his hand. Thank
you!
